National release of DHBs Sentinel and Serious events

The Health Quality & Safety Commission has released their ‘Making health and disability services safer’ report today. This is the Commission’s sixth report and the eighth time DHBs and other health providers have publically reported details of serious and sentinel adverse events (SAE).

As part of this report, the details and analysis of serious and sentinel events reported by DHBs and other providers and the Commission’s progress, including quality improvement case studies for the year 1 July 2013 – 30 June 2014, are noted. Individual publically released DHB reports supplement the Commission’s report.

The focus is on learning, safety and quality improvement for the whole system when things don’t go to plan.
