You may be eligible for a free wellness check and other health support services if you lived or worked within 1,200 metres to the north, east or south of the former Ivon Watkins Dow factory in Paritutu, New Plymouth between 1962 and 1987. Eligibility forms are available at all District Councils, Libraries, Doctors’ Surgeries and on this website. You will need to have your completed form verified by a Justice of the Peace, before sending it to the Ministry of Health. The form will be assessed by the Ministry of Health eligibility panel (this will take approximately four weeks). If approved you will be notified by mail and this letter will direct you to see your GP for a free annual wellness check. From there you may be referred to other health services. You are eligible for this service if you:

• lived, worked, or went to school in the area shown below between 1962 and 1987
• lived in Paritutu for at least one year between 1962 and 1969 or five years between 1970 and 1987
• worked at IWD for at least six months between 1962 and 1987
• worked at New Plymouth Power Station, Port Taranaki or elsewhere in Paritutu for at least one year between 1962 and 1969 or five years between 1970 and 1987
• lived at the power station workers’ camp, to the north of the former Ivon Watkins Dow Factory, for at least six months between 1969 and 1974
• attended Moturoa Primary School for at least one year between 1962 and 1969 or five years between 1970 and 1987
