Before or during a stay in hospital, a patient will have contact with many different departments and staff members. Case Managers make sure your care progresses smoothly across all departments in the hospital and in the transition to the community. We aim to ensure you and your family receive full and consistent information and are available for you and your family to discuss any part of your care.
Case Managers work closely with other staff to improve the hospital's services to patients overall and coordinate communication between the patient, family members, GPs, other hospitals, ACC and health care agencies (normal Privacy Act considerations apply). If you need advice regarding community services or help at home after you leave hospital, they can assist with this too.
